When your loved one needs to move — closer to family, to better care, or to a specific facility — our platform searches a growing network of facilities to identify potential matches. The platform helps manage timing and paperwork so families can focus on what matters.
How is this different from calling facilities directly?
When you call facilities individually, you're limited to whoever happens to have an open bed at that moment. CareSwaps uses a swap model — our algorithm identifies situations where two or more facilities can simultaneously exchange residents, creating openings that might not otherwise be visible. We also find multi-facility "swap chains" that create cascading opportunities across the network.
Does CareSwaps choose a facility for my loved one?
No. CareSwaps ranks matching options using operational factors — geographic proximity, bed availability, payer acceptance, and timing — plus any facility you specifically ask us to prioritize. CareSwaps shows facilities that participate in our network (not a comprehensive directory), and does not use clinical information to choose or recommend facilities; all clinical decisions about whether a transfer is appropriate are made independently by the licensed clinical staff at each facility. We provide the technology tools; facilities and families make the decisions.
What does the $299/month cover?
Your Platform Access Subscription includes full access to our matching algorithm, swap monitoring dashboard, and compliance document tools. This is a technology platform subscription for software access — not a one-time fee or a per-transfer charge. You can cancel anytime with no cancellation fee.
How long will I be paying $299/month?
For as long as it's useful to you. CareSwaps is month to month, and you can cancel anytime. It's built to be a tool you use while you're actively looking, so many families keep it for a season rather than indefinitely, much like a job-search membership you keep while you're searching and cancel once you're settled. Your subscription is for access to the matching platform. It is never a fee for a placement or transfer, and nothing you pay is tied to whether or not a move happens.
What if CareSwaps can't find a match?
Transfer outcomes depend on facility availability, clinical appropriateness, and regulatory requirements — factors outside our control. We can't guarantee a specific transfer or timeline. If you're within 7 days of your first payment, you can cancel for a full refund. Is my family's information safe?
Yes. CareSwaps operates as a HIPAA Business Associate and signs a BAA with each participating facility. All patient data is stored exclusively with BAA-covered service providers and transmitted via encrypted channels — we never store patient health information in non-BAA systems. Your data is protected with encryption, access controls, and audit logging.
Does CareSwaps handle transportation?
CareSwaps does not provide, coordinate, dispatch, or arrange transportation. You contract directly with the transport company of your choice — or you can arrange your own transportation. CareSwaps is a technology platform, not a transportation provider.
What states do you serve?
CareSwaps is now live in Colorado, with plans to expand to additional states soon after. Our facility network is growing across Colorado's 1,100+ senior care facilities — including skilled nursing, assisted living, memory care, CCRCs, and independent living communities.
